Document Upload Guidelines
This page explains which documents help GDCS review a dental insurance situation and what information should not be uploaded.
Helpful Documents
Helpful documents may include the EOB, denial letter, dental office bill, treatment estimate, pre-treatment estimate, claim form, receipt, itemized statement, plan benefit excerpt, reimbursement instructions, and any relevant payer or provider note.
For out-of-country or travel dental treatment, itemized receipts, procedure codes, provider details, claim forms, and plan language may be especially important.
Information To Keep Visible
When possible, keep visible the procedure codes, dates of service, tooth numbers if applicable, submitted charges, allowed amounts, paid amounts, deductibles, coinsurance, annual maximum notes, patient responsibility, claim number if needed, and EOB remark language.
Those details help GDCS explain what appears to have happened.
Information To Remove or Cover
Do not upload Social Security numbers. You may remove or cover unrelated account numbers, unrelated medical information, or other sensitive details that are not needed to understand the dental insurance issue.
