These terms describe the basic rules for using GDCS document-based dental insurance review services.

Service Description

GDCS provides document-based dental insurance clarity review. The service explains what submitted documents appear to show, identifies possible claim or coverage issues, and gives next-step guidance based on the documents provided.

Payment confirms the request for a document-based review. Payment does not guarantee insurance payment, reimbursement, claim approval, denial reversal, appeal success, savings, or any payer or provider action.

Your Responsibilities

You are responsible for submitting accurate documents and information. GDCS can only review what is provided.

Do not submit false documents, documents you are not authorized to share, Social Security numbers, or information that is not needed for the review.

Limits of Service

GDCS does not provide dental, medical, legal, financial, or insurance-broker advice. GDCS does not file claims, submit appeals, negotiate bills, contact providers or insurers on your behalf, or make treatment decisions.

The dental office, insurance plan, payer, provider contract, and applicable plan documents control the final outcome.

Refund and Clarity Guarantee

GDCS guarantees a good-faith, plain-language document review based on the information submitted. If GDCS cannot provide a meaningful clarity summary from the documents provided, GDCS may ask for additional information or refund the review fee.

This clarity guarantee applies only to the GDCS written explanation. It does not guarantee insurance payment, reimbursement, denial reversal, appeal success, savings, payer action, provider action, or any specific outcome.
